Water Quality and Your Health: Common Mistakes and What the Science Shows

Water is the most essential nutrient for human life, but the quality of the water we drink — and the relationship between water quality and long-term health — receives remarkably little attention in mainstream nutritional education. Most nutritional guidance focuses on what to eat, not on the quality of the water that supports every metabolic process in the body. Yet the evidence that water quality matters for health is substantial and, in parts of the world with water safety challenges, life-defining.

Mistake 1: Assuming Tap Water Is Nutritionally Identical Everywhere

The lesson: Tap water quality varies enormously between locations. The mineral content of water — calcium, magnesium, fluoride, chloride, sulphate — is determined by the geology of the aquifer or surface water source, the treatment process, and the distribution infrastructure. “Hard water” (high in dissolved calcium and magnesium) is associated in multiple ecological studies with lower rates of cardiovascular disease compared to “soft water” areas, with the proposed mechanism being the meaningful dietary contribution of water-borne magnesium and calcium. Studies from Finland, Sweden, the UK, and the US consistently find inverse relationships between water hardness and cardiovascular mortality. Conversely, areas with naturally soft water may have slightly higher rates of magnesium deficiency at the population level.

The improvement: Find out whether your tap water is hard or soft and what its mineral profile is. Local water suppliers publish detailed water quality analyses. If you live in a soft water area and are concerned about mineral adequacy, this is relevant context for dietary planning. If you are filtering tap water (with a reverse osmosis filter, for example), be aware that this removes minerals alongside contaminants and consider whether the filtered water profile is appropriate for your nutritional needs.

Mistake 2: Assuming All Bottled Water Is Better Than Tap Water

The lesson: In countries with high-quality water treatment and distribution infrastructure, there is typically no evidence that bottled water is healthier than tap water — and considerable evidence that bottled water has significant environmental disadvantages. The plastic bottles leach microplastics and phthalate compounds into the water over time and in high-temperature conditions, and microplastic contamination has been found in bottled water at significant levels. The plastic waste from single-use water bottles is an environmental and health problem in itself. In low-income countries or regions with compromised water infrastructure, bottled water may be genuinely necessary for safety — a context in which the environmental cost is outweighed by the infectious disease risk of contaminated tap water.

The improvement: In countries with reliable municipal water quality, use a glass or stainless steel reusable bottle filled with tap water. If taste or local contaminant concerns justify filtration, use a high-quality countertop or under-sink filter rather than single-use bottles. If you use a filter, replace the filter media on the manufacturer’s schedule — a clogged or overdue filter may actually harbour bacteria if not maintained.

Mistake 3: Underestimating the Global Water Safety Crisis

The lesson: Safe drinking water is not a universal given. The WHO estimates that approximately 2 billion people worldwide drink water contaminated with faeces, and that contaminated water causes approximately 485,000 deaths from diarrhoeal diseases annually — predominantly in children under five in low-income countries. Arsenic contamination of groundwater, affecting Bangladesh, West Bengal, parts of South and Southeast Asia, and parts of Latin America, causes chronic exposure that increases risk of cancer, cardiovascular disease, and neurological harm in tens of millions of people. Lead contamination from old pipe infrastructure — most famously evidenced in Flint, Michigan, but present in many older urban areas in high-income countries as well as in low-income countries — has irreversible neurodevelopmental consequences in children exposed during critical developmental periods. These are not edge cases of the water quality story. They are its central chapters for a significant proportion of humanity.

The improvement: Safe water is a fundamental human right, recognised as such by the United Nations in 2010. Advocacy for universal access to safe water — through investment in infrastructure, pollution control, and equitable water governance — is not separate from health advocacy. It is among the highest-priority public health investments available, with a return on investment from reduced infectious disease, reduced developmental harm, and improved nutritional status (since many waterborne contaminants directly impair nutrient absorption and metabolism) that is among the most compelling in global health.

Mistake 4: Overlooking Fluoride — Both in Its Absence and Its Excess

The lesson: Fluoride is a naturally occurring mineral that at low concentrations (approximately 0.7 to 1 mg per litre, the concentration used in water fluoridation programmes) reduces dental caries (cavities) by approximately 25 percent in children and adults — one of the most cost-effective public health interventions in oral health. Dental caries are not a cosmetic issue: they cause significant pain, impair eating and nutrition, are associated with systemic infections in severe cases, and are disproportionately prevalent in lower-income communities without access to dental care. At high concentrations (above 1.5 mg/litre), fluoride causes dental fluorosis (white spots on teeth, primarily cosmetic) and at very high concentrations (above 4 mg/litre, found naturally in some groundwaters in India, China, and East Africa) causes skeletal fluorosis — a crippling bone disease affecting millions of people in endemic regions who have no practical alternative water source.

The improvement: In communities without water fluoridation, ensure dental fluoride exposure through toothpaste (using fluoride toothpaste is the most impactful single oral health measure for most people). In communities with elevated natural fluoride (above 1.5 mg/litre), water defluoridation or alternative water sources are important health interventions.

Mistake 5: Not Connecting Water Access to Nutritional Outcomes

The lesson: The relationship between water quality and nutrition is more direct than the separate treatment of these topics in public health implies. Safe water is required for food preparation, including cooking, washing produce, and hydrating dried legumes and grains that are the staple foods of many low-income populations. In households without safe water, these food preparation steps carry infectious disease risk that directly undermines nutritional outcomes. Children with recurrent diarrhoeal illness from contaminated water have persistently impaired nutrient absorption and are at significantly higher risk of malnutrition, stunting, and micronutrient deficiency, independent of what they eat. Addressing water quality and nutritional adequacy simultaneously — as integrated components of the right to food and health — produces dramatically better outcomes than addressing either in isolation. Every nutrition programme operating in water-unsafe contexts that does not also address water safety is missing a critical component of the problem it is trying to solve.
